What is closed rhinoplasty?

In closed rhinoplasty, all incisions are made inside the nostrils, leaving no visible external scarring. The surgeon works through these internal incisions to access and reshape the underlying bone and cartilage.

This technique is typically used for less complex cases — such as reducing a dorsal hump, refining the nasal tip modestly, or making straightforward adjustments to the nasal bones. Because the nasal skin is not lifted away from the underlying structure, closed rhinoplasty is often associated with somewhat less swelling and a shorter initial recovery period.

What is open rhinoplasty?

In open rhinoplasty, the surgeon makes a small additional incision across the columella — the thin strip of tissue between the nostrils — in addition to the internal incisions. This allows the skin covering the nose to be gently lifted, giving the surgeon direct, unobstructed visibility of the entire nasal structure.

Open rhinoplasty is generally preferred for more complex cases: significant tip reshaping, correcting asymmetry, revision rhinoplasty, or cases involving cartilage grafting. The trade-off is a small external scar along the columella, which in the vast majority of cases fades to become barely noticeable within several months.

Which technique is right for you?

The right technique depends entirely on what you’re looking to achieve. As a general guide:

Closed rhinoplasty tends to be well suited for patients who need modest bone reshaping, dorsal hump reduction, or straightforward nasal bridge adjustments — including cases combined with our ultrasonic rhinoplasty technique.

Open rhinoplasty tends to be the better choice for patients requiring significant tip refinement, correction of asymmetry, revision of a previous rhinoplasty, or cases where cartilage grafting is needed to support the nasal structure.

Neither technique is inherently “better” — they are different tools suited to different surgical goals. An experienced rhinoplasty surgeon will recommend the technique that best serves your specific anatomy and desired outcome, not default to one approach for every patient.

Does the technique affect recovery time?

Both techniques follow a broadly similar recovery timeline. A splint is worn for approximately two weeks in both cases, and the majority of visible swelling resolves within the first month. Closed rhinoplasty patients sometimes report slightly less initial swelling since the skin is not lifted from the underlying structure, but by the six-week mark, most patients — regardless of technique — are at a similar stage of recovery.
